PCOS is also associated with altered signaling in the HPO axis, particularly an increased luteinizing hormone (LH) to follicle-stimulating hormone (FSH) ratio. This imbalance further disrupts ovulation.

PCOS presents heterogeneously, meaning no two individuals experience it exactly the same way. Common features include:
Menstrual Irregularities
Oligomenorrhea (infrequent periods)
Amenorrhea (absence of periods)

Hirsutism (face, chest, abdomen)
Moderate to severe acne
Androgenic alopecia
Metabolic Features

Acanthosis nigricans (darkened skin patches, often on neck or axilla)

Cardiovascular Disease
Individuals with PCOS may have:
Dyslipidemia (abnormal cholesterol levels)

Irregular or absent ovulation leads to prolonged estrogen exposure without progesterone, increasing the risk of endometrial thickening and malignancy.
